What Our Medical Records Department Does
- Patient record creation and maintenance — generating and maintaining complete, accurate medical records for every OPD, IPD, emergency, and surgical patient
- Medical documentation management — organising discharge summaries, operation notes, investigation reports, consent forms, and clinical correspondence
- Record retrieval — providing treating doctors and authorised staff with prompt access to patient history when needed for ongoing care
- Medico-legal documentation — preparing certified medical records, fitness certificates, and medico-legal case (MLC) documentation as required by law or court orders
- Record confidentiality — ensuring patient records are accessible only to authorised personnel in compliance with patient privacy norms
- Birth and death registration — maintaining statutory registers and issuing birth and death certificates as per regulatory requirements
- Record retention and archiving — storing physical and digital records for the legally required retention period
