The Medical Records and IT Department at IPIMS Hospital ensures that this information is accurately maintained, securely stored, and readily available — to the right person, at the right time.

What Our Medical Records Department Does

  • Patient record creation and maintenance — generating and maintaining complete, accurate medical records for every OPD, IPD, emergency, and surgical patient
  • Medical documentation management — organising discharge summaries, operation notes, investigation reports, consent forms, and clinical correspondence
  • Record retrieval — providing treating doctors and authorised staff with prompt access to patient history when needed for ongoing care
  • Medico-legal documentation — preparing certified medical records, fitness certificates, and medico-legal case (MLC) documentation as required by law or court orders
  • Record confidentiality — ensuring patient records are accessible only to authorised personnel in compliance with patient privacy norms
  • Birth and death registration — maintaining statutory registers and issuing birth and death certificates as per regulatory requirements
  • Record retention and archiving — storing physical and digital records for the legally required retention period
